Our membership is not a dental insurance plan and is only valid at Mor Smiles. It is non-transferable and it cannot be combined with any other dental insurance coverage. Discounts under your membership DO NOT apply to any other treatment prior to joining. Initial 12 month enrollment is required. Must enroll for one year to receive benefits. You must pay first year’s membership fee & lifetime enrollment fee in full upfront. Plan will automatically renew on the 13th month unless you provide Mor Smiles with a written statement detailing that you will be canceling the contract. Cancellation terms will go into effect 30 days after cancellation letter has been received. If you default on your payment plan, all discounts will be removed from your account and you will be responsible for the total balance in full.
